Vitamin-D-Rezeptor-Aktivierung mit deutlich verringerter Hyperkalzämiegefahr - Vorteil Selektivität

Normalerweise wird Vitamin D als chemische "Vorstufe" im Darm aufgenommen, zum Teil auch in der Haut unter Lichteinwirkung gebildet. In Leber und Niere wird es dann aktiviert (man spricht daher auch von aktivem Vitamin D, andere gängige Bezeichnungen sind Calcitriol, oder auch Vitamin-D-Hormon). Ist genügend Calcitriol vorhanden, werden die Vitamin-D-Rezeptoren (VDR) an Darm- und Knochenzellen aktiviert, was zur enteralen Kalziumresorption und zum Knochenaufbau führt. Im Rahmen einer chronischen Niereninsuffizienz (CKD) kommt es allerdings recht schnell zum Absinken der renalen Calcitriolbildung und folglich zu erniedrigten Calcitriolserumspiegeln. Bereits 30-50 % der Patienten in den CKD-Stadien 2 und 3 weisen einen Calcitriolmangel auf [1].
