Wie gehe ich im Alltag vor ? Prophylaxe– und Therapiealgorithmen

Zusammenfassung

Postoperative Übelkeit und postoperatives Erbrechen (PÜ&E) sind für den Patienten gleichbedeutend mit dem Auftreten postoperativer Schmerzen. Folglich sollte eine konsequente und wirksame Prophylaxe ebenso zum anästhesiologischen Vorgehen gehören wie die perioperative Schmerztherapie. Auch wenn keines der gegenwärtig verfügbaren Antiemetika das Problem zu eliminieren in der Lage ist, kann durch die Nutzung additiver Effekte bei Kombination mehrerer Interventionen in der Regel auch bei Hochrisikopatienten eine zufriedenstellende Risikoreduktion erzielt werden. Für wirksame Einzelinterventionen gilt, dass sie approximativ mit einer 30 %igen relativen Risikoreduktion assoziiert sind. Zu diesen Antiemetika gehört beispielsweise die Gabe von 4mg Ondansetron, 4mg Dexamethason, 1,25mg Droperidol, 62mg Dimenhydrinat sowie das seit einiger Zeit verfügbare orale Aprepitant. Auch Metoclopramid soll nicht unerwähnt bleiben und kann z.B. im Rahmen von multimodalen Ansätzen nutzbringend eingesetzt werden. Neben der Gabe von Antiemetika ist ein vergleichbarer Effekt auch durch die Modifikation des anästhesiologischen Vorgehens zu erzielen, d.h. durch Verzicht auf Inhalationsanästhetika und Verwendung von Propofol. Grundsätzlich hilft ein risikoadaptiertes Vorgehen eine unnötige Antiemetikagabe bei Patienten mit niedrigem Risiko zu vermeiden. Aufgrund einer erschwerten Implementierung und den Unsicherheiten, die mit der Risikoprädiktion für individuelle Patienten verbunden sind, scheint ein großzügiger genereller Einsatz von Antiemetika unter Umständen vorteilhafter. Dies umso mehr, als die meisten der verfügbaren Antiemetika ein günstiges Nutzen–Nebenwirkungs–Verhältnis aufweisen.

Abstract

Postoperative nausea and vomiting (PONV) constitutes a major unpleasant symptom in the postoperative period. The prevention of PONV is judged equally important as the prevention of postoperative pain. Therefore, a working PONV–algorithm should be as self–evident as the approach to prevent and treat postoperative pain. None of the currently available pharmacological interventions is able to totally abolish PONV. However, using a multimodal approach with the combination of various antiemetic interventions, a substantial reduction or even elimination of PONV is already feasible. As a rule of thumb, each effective antiemetic intervention will lead to a relative risk reduction of approximately 30 %. Well documented interventions in terms of the aforementioned efficacy are the aministration of ondansetron 4mg, dexamethasone 4mg, droperidol 1,25mg and dimenhydrinate 62mg, as well oral Aprepitant. Metoclopramide may play a role for instance in a multimodal approach. Apart from the administration of antiemetics, the avoidance of inhalational anaesthetics by using propofol is associated with a comparable risk reduction. In general, using a risk–dependent approach, e.g. based on a simplified risk score, allows to avoid administering antiemetics to patients at low risk. However, due to the difficulties associated with the implementation of risk–score based algorithms and the inherent weaknesses of clinical risk scores to predict PONV in an individual patient, a general (multimodal) approach seem to be justified as well. Considering the fact that the currently available antiemetics are associated with few side effects, the administration of prophylactic antiemetics should not be associated with a high hurdle in the clinical setting. In case of any doubts regarding the individual risk, it seems justified to expand the (multimodal) prophylaxis rather than to wait until PONV occurs and impairs patient comfort.

Kernaussagen

• Postoperative Übelkeit und postoperatives Erbrechen (PÜ & E) sind für den Patienten ebenso bedeutsam wie postoperative Schmerzzustände.

• Die Vermeidung bzw. Therapie von PÜ & E sollte deshalb ebenso stringent und konzertiert verfolgt werden wie die Behandlung postoperativer Schmerzen.

• Die Modifikation des anästhesiologischen Vorgehens (z. B. Propofol–TIVA statt Inhalationsanästhesie) hat vergleichbare Effekte auf die Inzidenz von PÜ & E wie die Gabe einer effektiven Dosis eines Antiemetikums.

• Ondansetron (4 mg), Dexamethason (4 mg), Droperidol (1,25 mg) und Dimenhydrinat (62 mg) können als vergleichbar effektive Interventionen angesehen werden.

• Eine Einzelintervention vermag das Risiko für PÜ & E um etwa ein Drittel zu senken (ca. 30  % ige relative Risikoreduktion).

• Die Kombination mehrere Interventionen aus unterschiedlichen Substanzklassen bringt additive Effekte.

• Aufgrund der limitierten Effektivität derzeit verfügbarer Einzelsubstanzen ist – insbesondere bei Hochrisikopatienten – ein multimodaler Ansatz (d. h. eine Kombination von 2, 3 oder mehr Interventionen) vonnöten.

• Zur Therapie sollten Antiemetika aus einer anderen Substanzklasse als bei der Prophylaxe verwendet werden, vor allem, wenn die Prophylaxe weniger als 6 Stunden zurückliegt.

• Der grundsätzliche Vorteil einer risikoadaptierten Vorgehensweise liegt in der Einsparung von Antiemetika bei Patienten mit niedrigem Risiko (die wahrscheinlich nicht unter PÜ & E leiden würden).

• Für eine fixe Kombinationsprophylaxe gegenüber einem risikoadaptierten Algorithmus spricht: Die Vorhersage des individuellen Risikos ist unzuverlässig und die Implementierung eines risikoadaptierten Algorithmus schwierig (individuelle Adaptation der Therapie auf die Prophylaxe erforderlich, Akzeptanzprobleme, Vergessen etc.).

• Die Therapie sollte zügig mit zeitnaher Erfolgskontrolle (und ggf. einer Erweiterung der Maßnahmen) erfolgen.

• Multimodale Ansätze, z. B. die Kombination eines rasch wirkenden (Ondansetron) mit einem lang wirkenden Antiemetikum (Dexamethason), sind geeignet, um die Therapie effektiver zu gestalten.

• Standardverfahren oder SOPs erleichtern die Implementierung und fördern die notwendige Durchdringung im klinischen Alltag.
