Endoskopische Behandlung der Pankreatitis und ihrer Komplikationen

Kernaussagen

• Bei schweren Verlaufsformen der Pankreatitis gibt es neben der Endoskopie und der Chirurgie nur wenige Therapieansätze.

• Die endoskopische Behandlung versucht, die normale Anatomie zu erhalten oder zu rekonstruieren, Infektionen auszuräumen und Obstruktionen zu beseitigen.

• Ein Therapieerfolg kann von ca. 50 % langfristig erwartet werden. Insgesamt gibt es nur wenige Studien, die eine fundierte Aussage zum Vorgehen im Detail und im Vergleich zur Chirurgie bei unterschiedlichen Konstellationen erlauben.

• Zurzeit werden zystische Läsionen und infizierte Nekrosen in Zentren zunehmend transmural endoskopisch behandelt.

• Bei der stenosierenden chronischen Pankreatitis zeigen zwei randomisierte Studien eine Überlegenheit der Chirurgie, aber auch akzeptable Erfolgsraten der Endoskopie. Deshalb erscheint ein primärer endoskopischer Therapieversuch in den meisten Fällen gerechtfertigt. Bei komplizierteren Fällen muss die Therapieentscheidung dem Einzelfall angepasst werden.
