Staging

  • Um die richtige Auswahl von für eine kurative endoskopische Therapie geeigneten Patienten treffen zu können, ist ein genaues initiales Staging einschließlich hochauflösender Endoskopie mit Chromoendoskopie und Endosonografie erforderlich.

Techniken und Indikationen

  • Die endoskopische Resektion (ER) ist bei frühen Neoplasien eine effektive und sichere Alternative zu Ösophagusresektion bzw. Gastrektomie, wenn keine Risikofaktoren für eine Lymphknotenmetastasierung vorliegen.

  • Die Suck-and-Cut-Technik unter Anwendung einer Kappe oder eines Ligatursystems ist die Standardtechnik für die ER früher Neoplasien. Läsionen über 20 mm Größe werden in Piecemeal-Technik abgetragen.

  • Die endoskopische Therapie von Frühneoplasien sollte in Analogie zu chirurgischen Standards in Zentren mit entsprechender Expertise erfolgen („High-Volume-Zentren”).

  • Aufgrund des Risikos metachroner Läsionen und von Rezidiven sollten die Patienten gemäß einem standardisierten Nachsorgeprotokoll nachbeobachtet werden. In der Regel ist eine effektive endoskopische Therapie auch von Zweitläsionen möglich.
