Kombinierte Chemotherapie und regionale Tiefenhyperthermie eines hormonrefraktären, ossär und hepatisch metastasierenden Prostatakarzinoms in der 5. Therapielinie

Zusammenfassung

Als systemische Standardtherapie des hormonrefraktären Prostatakarzinoms hat sich seit etwa 2003 die Chemotherapie mit Docetaxel zunehmend etabliert. Basis dafür waren die Ergebnisse der TAX 327 Studie [2], die erstmals einen statistisch signifikanten Überlebensvorteil für die mit Docetaxel behandelten Patienten dokumentierte. Dieser Vorteil wurde auch durch aktualisierte Analysen, zuletzt 2007 [2], nachhaltig bestätigt. Bis zur Etablierung des Taxans in die Erstlinientherapie galt das hormonrefraktäre Prostatakarzinom (HRPC) als chemotherapeutisch sehr schwierig zu behandeln. Eine effektive und etablierte 2.-Linientherapie nach Versagen oder Progression unter Docetaxel existiert derzeit nicht. Auch aus dem Bereich der zahlreichen neuen molekularen Therapieverfahren steht momentan für das Prostatakarzinom keine Substanz zur Verfügung. In dieser Fallbeschreibung wird der ungewöhnlich erfolgreiche Behandlungsverlauf bei einem Patienten mit HRPC vorgestellt. Neben verschiedenen Zytostatika wurden in den letzten fast 3 Jahren der Behandlungszeit des Patienten auch ausgewählte Behandlungsmethoden aus dem Bereich der komplementären Onkologie eingesetzt.

Summary

Since 2003 Docetaxel seems to be widely accepted as systemic standard therapy for hormone refractory prostate cancer. This is due to the results of the TAX 327 crossover study [2], that documented a statistic significant prolongation of survival and symptom control, for the first time. Before the introduction of this new standard, chemotherapy of hormone refractory prostate cancer was considered without real benefit. For therapy failure under treatment with docetaxel, no established salvage therapy is available at the moment. This case report presents the course of disease and treatment of a patient with metastasizing prostate cancer until 5th line chemotherapy combined with several complementary methods.
