Einleitung

In Deutschland ist ein Rückgang der Bevölkerung zu verzeichnen. Laut einer Veröffentlichung des Berlin-Instituts für Bevölkerung und Entwicklung nimmt bei anhaltend niedriger Geburtenrate, jedoch steigender durchschnittlicher Lebenserwartung die negative demografische Entwicklung Deutschlands an Geschwindigkeit immer weiter zu. Die Folge ist eine Gesellschaft mit einem immer höheren Durchschnittsalter. Die Lebenserwartung hat sich in den letzten 100 Jahren nahezu verdoppelt. Deshalb wird sich die Gruppe der über 80-Jährigen in den nächsten 50 Jahren verdreifachen, von heute 3,2 Millionen (3,9 % der Bevölkerung) auf 9,1 Millionen (12,1 %). Die Gruppe der über 100-Jährigen wird sogar 16-mal so groß werden (2000: 7200, 2050: 114 700) [[1]].

Trotz aller Bemühungen der Prophylaxe steigt durch die Summation der durch Karies, Trauma und Parodontopathien verloren gegangenen Zähne der prothetische Behandlungsbedarf mit zunehmendem Lebensalter an. Jedoch ist dank der Erfolge der Prophylaxe eine Verschiebung der notwendigen Behandlungsmittel eingetreten: War gestern die Totalprothese und ist heute der herausnehmbare Zahnersatz das therapeutische Medium der Wahl, so wird in Zukunft diese Verschiebung zum festsitzenden Zahnersatz erfolgen. Probleme entstehen dabei nicht so sehr durch den Zerstörungsgrad der Zähne, sondern durch den ubiquitär anzutreffenden reduzierten Parodontalzustand.

Somit wird die Gerostomatologie zunehmend an Bedeutung gewinnen. Dabei ist das Feld der Alterszahnheilkunde sehr weit: Es erstreckt sich von kleineren konservierenden Maßnahmen – etwa der Versorgung von Zahnhalsdefekten (Abb. [1]) – bis hin zur totalprothetischen Versorgung und oralhygienischen Betreuung solcher Patienten, die zu einer eigenen, effizienten Mundpflege und/oder Prothesenhygiene nicht mehr in der Lage sind (Abb. [2]).
