Zusammenfassung
Intraduktal papillär-muzinöse Tumore (IPMT) des Pankreas sind intraduktal gelegene,
Muzin-produzierende, zystische Tumore des Pankreas mit malignem Potenzial. Wir berichten
über den Fall einer 54-jährigen Frau, die aufgrund eines nicht-invasiven IPMT einer
Pankreassegmentresektion unterzogen wurde. Die Resektatränder des Operationspräparates
waren tumorfrei. Drei Jahre später fiel im Rahmen einer Nachuntersuchung eine hochgradig
malignitätsverdächtige Raumforderung im Pankreasschwanz auf, woraufhin eine Pankreasschwanzresektion
durchgeführt wurde. Histologisch zeigte sich ein Adenokarzinom. Anzunehmen ist in
diesem Fall die Entwicklung eines Pankreaskarzinoms auf dem Boden eines nicht-invasiven
IPMT. Dieser Fall wirft erneut die Frage auf, wie ausgedehnt nicht-invasive IPMT des
Pankreas reseziert werden sollen.
Abstract
Intraductal papillary mucinous neoplasms (IPMNs) of the pancreas are intraductally
located, mucin-producing cystic neoplasms of the pancreas with a malignant potential.
We report about a 54-year-old female who underwent segmental resection of the pancreas
for non-invasive IPMN. The surgical margins were tumour-negative. Three years later
a highly suspicious tumour of the pancreatic tail was detected during routine follow-up.
Resection of the pancreatic tail was performed. The histological analysis revealed
an adenocarcinoma. This case suggests the development of a pancreatic carcinoma from
a non-invasive IPMN and raises the question about the extent of surgery of non-invasive
IPMNs of the pancreas.
