Pharmakologische und galenische Einflüsse auf Wirkdauer und analgetische Wirksamkeit

Zusammenfassung

Retardierte Opioide bilden einen wichtigen Baustein in der Tumorschmerztherapie. Die 3 in Deutschland zugelassenen Opioide der WHO-Stufe III, Morphin, Oxycodon und Hydromorphon haben alle eine starke analgetische Wirksamkeit. Doch hinsichtlich aktiver Metaboliten, Interaktionsprofil, Wirkung auf das P-Glykoprotein sowie das pharmakokinetische Profil bestehen eine Reihe von relevanten pharmakologischen Unterschieden. Bei der Auswahl der Opioide gilt es insbesondere, die Halbwertsdauer als auch das Phänomen des Dose-Dumpings zu berücksichtigen.

Abstract

Sustained-release opioids are an important element in therapy of tumor pain. All of the three opiods available in Germany – morphine, oxycodone and hydromorphone – are having a strong analgesic efficacy. But there are certain pharmacologic differences with regard to the active metabolites, the interaction profile, the effect on p-glycoprotein and the pharmacokinetic profile. When choosing opioids the half-value duration and the phenomenon of dose-dumping are further criterias to be taken into consideration.
