Symptomatik und klinische Diagnostik der Panarteriitis nodosa

Zusammenfassung

Bei fünf Patienten wurde die Diagnose einer Panarteriitis nodosa intra vitam gestellt; zwei starben innerhalb kurzer Zeit, drei werden jetzt bis zu vier Jahren beobachtet. Klinische Symptome und klinisch-chemische Befunde müssen den ersten diagnostischen Hinweis auf eine Panarteriitis geben. Der Beweis ist schließlich durch eine Probebiopsie aus einem befallenen Organ zu führen. Die Prognose ist unterschiedlich und im Einzelfall nur mit Vorsicht zu stellen. Art, Ausmaß und Anzahl befallener Organe spielen prognostisch eine entscheidende Rolle. Als Ursache wird eine allergisch-hyperergische Reaktion der Gefäßwand angenommen. Zur Therapie kommen Corticosteroide und Immunsuppressiva in Betracht. Auf diese Medikation spricht die neuromuskuläre Form der Panarteriitis recht gut an, die Prognose hängt jedoch davon ab, ob gleichzeitig eine Viszeralmanifestation besteht.

Summary

Panarteritis nodosa was diagnosed in five patients, two of whom died shortly thereafter, while three are followed now up to four years. Clinical and biochemical findings will give the first diagnostic pointer to panarteritis nodosa, while the only proof can be by biopsy of the affected organ. Prognosis varies and must be assessed cautiously in the individual case. Type, extent and number of organs involved are of decisive role prognostically. An allergic-hyperergic reaction of the vessels walls is thought to be the cause. Corticosteroids and immunosuppressives may be used therapeutically. The neuromuscular form of panarteritis responds quite well to this form of medication, but prognosis depends on whether there is also visceral involvement.
