Summary
Endoscopic figures on the growth of gastric cancer are selected from our permissive long-term gastrophotographic follow-up cases. Histopathological changes in these cases are finally identified microscopically together with their macroscopical course demonstrated and illustrated. The gastrophotographic appearance on some stages during follow-up period are compared and differential diagnostic possibilties on those figures discussed. Finally a speculated monograph on the fate of polypoid gastric lesion and ulcerative gastric lesion are schematically illustrated. It is concluded that: 1. a benign polyp seldom malignantly changes to become a polyp cancer; 2. a polyp cancer is able to dissect spontaneously from the gastric mucosa as a benign polyp does: 3. polypoid cancer is seldom developed from the polyp cancer but it is frequently originated from a flat malignant focus; 4. benign ulcer relapses frequently but seldom undergo malignant changes; 5. malignant ulcer also relapses in its natural course; 6. the growth and infiltration of gastric cancer is found to be gradual but it is exaggerated due to the vascular infiltration of the malignancy.
Zusammenfassung
Endoskopische Verlaufsbeobachtungen an Hand von Gastrophotogrammen lassen folgende Schlußfolgerungen zu: 1. ein benigner Polyp wird nur selten maligne; 2. auch ein polypös wachsendes Karzinom kann sich spontan amputieren wie ein benigner Polyp; 3. ein polypoides Karzinom geht selten auf einen verkrebsten Polypen zurück, sondern nimmt eher seinen Ausgang von einer flachen malignen Läsion; 4. benigne Ulzera rezidivieren häufig, eine maligne Degeneration ist hingegen nur selten zu beobachten; 5. auch beim malignen Ulkus kommt es zu Abheilung und Rezidiv; 6. Wachstum und Infiltration beim Magenkarzinom gehen nur allmählich vonstatten; bei einer Gefäßinfiltration ist jedoch eine rasche Progredienz zu beobachten.
