Klinische und hämodynamische Untersuchungen über die Brauchbarkeit der Starr-Edwards-Diskusklappen in Mitralposition

Zusammenfassung

Die mechanische Zuverlässigkeit der Starr-Edwards-Diskusklappen (Modelle 6520 und 6550) wird durch eine Reihe klinischer und hämodynamischer Nachteile aufgewogen. Das zeigt eine retrospektive Untersuchung an 205 Patienten 3-67 Monate (m = 30,4) nach isoliertem Mitralklappenersatz. Am meisten gravierend erwiesen sich hohe Hämolyserate, funktionell unzureichende Klappenöffnungsfläche und Fortbestand frequenz- und volumenabhängiger Druckgradienten. Die Thromboemboliehäufigkeit war dagegen anderen Kunstklappen vergleichbar.

Summary

205 patients with isolated mitral valve replacement using the Starr-Edwards low profile valve prostheses (model 6520 and 6550) were followed 3 to 67 months mean of 30,4 months) after surgery. While these prostheses seem to be reliable substitutes with regard to functional performance and to mechanical dysfunction, postoperative hemodynamic investigation revealed significant diastolic pressure gradients across the valve as well as reduction in functional mitral valve area relative to pulse rate and stroke volume. The incidence of thromboembolism was comparable to other artificial valves.
