Aneurysma der A. carotis int. im extrakraniellen Verlauf

Zusammenfassung

Beschrieben wurde der Fall eines Aneurysmas im extrakraniellen Verlauf der A. carotis int., welches sich nach einem Trauma des Schädels bei Fraktur des Unterkiefers entwickelte. Wegen seiner hohen Lage und dem Operationsbefund konnte keine andere Methode gewählt werden und nach vorhergehendem Test des Kollateralkreislaufs wurde nur die Abbindung der A. carotis int. durchgeführt. Nach der Operation war der Patient ohne Anzeichen einer Blutinsuffizienz des Gehirns.

In der Diskussion beschäftigen sich die Autoren mit der Ätiologie, Symptomatologie und der Therapie dieser Aneurysmen und geben einen umfangreichen Überblick der Literatur, soweit sie sich mit diesem Thema befaßt.

Summary

The authors describe a case of aneurysm in the extracranial course of the internal carotid artery which developed after skull injury and fracture of the lower jaw. In view of the high localisation and surgical finding, it was not possible to use another therapeutic method and only ligature of internal carotid was performed. The patient after operation has no signs of an inadequate blood supply of the brain.

In the discussion the authors discuss the etiology, symptomatology and therapy of these aneurysms and present a vast survey of world literature which deals with this problem.

Résumé

Il's'agit d'un cas d'anévrisme de la carotide interne développé à la suite d'un traumatisme cranien avec fracture de la machoire inférieure. A cause de sa situation élevée et des constations opératoires on a dû se résoudre, après controle d'une circulation collatérale, a ligaturer la carotide interne. Après cette intervention ce malade n'a pas montré d'insuffisance vasculaire.

Les auteurs donnent un aperçu étendu de la littérature à ce sujet.

Resumen

Se describe un caso de aneurisma en el trayecto extracraneal de la arteria carótida interna, el cual se desarrolló después de un trauma craneal junto con fractura de maxilar inferior. Debido a su alto emplazamiento y al diagnóstico operatorio, no se pudo elegir ningún otro método y después de un test previo de la circulación colateral, sólo se llevó a cabo la ligadura de la arteria carótida interna. Después de la intervención el paciente no tuvo síntoma alguno de insuficiencia de riego cerebral.

Los autores discuten la etiología, sintomatología y terpaéutica de estos aneurismas y dan una visión general de la bibliografía en relación con el tema.
