Karotisangiographie nach Entwässerung – bessere Darstellung von Hirntumoren

Zusammenfassung

Es wurde an 13 Patienten die zerebrale Durchblutung vor und nach Infusion von 100 g Sorbit angiographisch untersucht. Zusätzlich wurde bei 2 Patienten die Hirndurchblutung mit der 133-Xenon-Clearance bestimmt. Wir sahen eine beschleunigte Hirndurchblutung bei Engstellung der Arterien und eine Steigerung des Durchblutungsvolumens pro Zeiteinheit um 30 bis 33%. Bei diesen Bedingungen war die Darstellung pathologischer Gefäβnetze erheblich verbessert. Es empfiehlt sich, die zerebrale Angiographie bei Patienten mit Hirndruck und Tumorverdacht unmittelbar nach einer Infusion von 100 g Sorbit durchzuführen.

Summary

In thirteen patients the cerebral circulation was studied by angiography before and after the infusion of 100 g. of Sorbitol. In addition in two patients the brain circulation was determined by the Xenon¹³³ clearance test.

We observed an acceleration of the circulation with narrowing of the arteries and an increase of circulating volume per unit of time of 30 to 33%. In these conditions there was marked improvement in the demonstration of the pathological circulation. In tumour suspects and patients with raised pressure it is recommended that angiography is performed immediately after an infusion of 100 g. of Sorbitol.

Résumé

Le débit cérébral a été examiné chez 13 malades après perfusion de 100 gr. de Sorbitol. Chez 2 autres malades, le débit fut déterminé selon la technique de la clearance au Xenon 133. Nous avons constaté une accélération du débit artériel avec rétrécissement du calibre artériel avec augmentation de 30 à 33% du volume débité par unité de temps.

Dans ces conditions, la visualisation du réseau vasculaire pathologique a été considérablement améliorée. Il est recommandé d'effectuer l'angiographie cérébrale chez les malades atteints d'hypertension intracranienne et suspects de tumeur cérébrale après perfusion d'une solution de 100 gr. de Sorbitol.

Resumen

Se ha estudiado en 13 pacientes la circulación cerebral antes y despues de la administración de una infusión de 100 grms. de Sorbit. Además se determinó en 2 enfermos la circulación intracerebral con la Xenon¹³³-Clearance. Hemos observado una aceleración de la circulación intracerebral y un aumento del volumen circulatorio de un 30–33%, por unidad de tiempo, junto con una disminución del calibre de las arterias. Bajo estas condiciones se consiguió una representación de redes vasculares patológicas considerablemente mejor. En pacientes con presión intracraneal y sospecha de tumor cerebral se recomienda realizar la angiografía cerebral después de la administración de una infusión de 100 grms. de Sorbit.
