Zusammenfassung

Anhand vier eigener Fälle wird darauf hingewiesen, daß ein ischiasähnliches Syndrom gelegentlich von einer Verdickung der Leptomeninx im Sinne der Leptomeningitis chronica fibroplastica im Bereich der Cauda equina hervorgerufen werden kann. Sowohl bei der zystischen als auch bei der fibrösen Form werden die Wurzeln der Cauda equina komprimiert. Die Entfernung der veränderten Leptomeninx unter dem Operationsmikroskop hat in den beschriebenen Fällen zufriedenstellende Resultate erzielt. Eine vollkommene Beschwerdefreiheit kann nur selten erreicht werden, da das pathologische Gewebe nie radikal entfernt werden kann.

Summary

An almost typical sciatic syndrome can be occasionally associated with a thickening of the leptomeninges, producing a type of chronic fibro-plastic meningitis in the region of the cauda equina. We report here four personal cases, which showed atypical myelographic patterns. The roots of the cauda equina were compressed both by the cystic and by the fibrosing types. In the cases described, the removal of the altered leptomeninges under the operating microscope produced a satisfactory result. A completely symptom-free outcome can rarely be achieved, as the pathological tissue cannot be completely removed.
