Zusammenfassung

Patientinnen mit einer Borderline-Persönlichkeitsstörung (BPS) stellen in der stationären Akut- und Krisenversorgung eine große Herausforderung dar. Der Beitrag beschreibt diagnostische und therapeutische Standards, Hinweise zur Einschätzung des Schweregrades der Störung und abzugrenzende Differenzialdiagnosen.

Abstract

Patients with borderline personality disorder (BPD) pose a major challenge in inpatient acute and crisis care. Severely and persistently ill patients in particular often require long and complex hospital stays. This paper describes diagnostic standards, and provides information on assessing the severity of the disorder and differential diagnoses. In addition to medication-based treatment strategies, tools from Dialectical Behavioral Therapy (DBT), Transference-focused Psychotherapy (TFP) and Good Psychiatric Management (GPM) are described that can be used in acute psychiatry. Treatment is based on a treatment agreement, which represents a jointly agreed framework and defines treatment goals. Intervention options from clinical practice are presented for the management of serious incidents, ranging from significant self-harm and aggression toward others to persistent dissociative states. Finally, the paper discusses the transition to a more stable treatment setting and outlines recommendations for establishing a supportive living environment for people with BPD.
