Transition in die Erwachsenenmedizin: bekannte Herausforderungen neu denken – von internationalen und digitalen Ideen

Zusammenfassung
Erfahrungen in der Arbeit mit Jugendlichen aus verschiedenen inner- und außereuropäischen Ländern zeigen, dass ein zentraler Aspekt der Übergangsphase der Wunsch nach einer Behandlung auf Augenhöhe ist. Die Jugendlichen möchten von ihren Ärzt*innen ernst genommen werden und wünschen sich, als Ganzes gesehen zu werden. Sie möchten nicht nur als Patient*innen mit einer Diagnose wahrgenommen werden, sondern als Individuen mit eigenen Lebensrealitäten und Herausforderungen, die sie neben ihrer Erkrankung zu bewältigen haben.
Dieses Leben umfasst nicht nur die regelmäßige Einnahme von Medikamenten und die Kontrolle von Blutwerten, sondern auch die Bewältigung von sozialen, emotionalen und schulischen Herausforderungen. Für die Umsetzung einer patientenzentrierten Behandlung gilt es, gemeinsame Fortbildungsprogramme für die Pädiatrie und die Erwachsenenmedizin durchzuführen. Die Lebenswirklichkeit von jungen Erwachsenen muss in der Betreuung der Internist*innen Bedeutung finden und in der Pädiatrie müssen wir die Jugendlichen in allen Lebensbereichen umfassend schulen, sodass sie der neuen Betreuungsstruktur gewachsen sind.
