Zusammenfassung
Das Pyoderma gangraenosum (PG) ist eine seltene Erkrankung, die durch eine sterile,
schmerzhafte und schnell progrediente Ulzeration mit einem livide gefärbten und unterminierten
Randsaum charakterisiert ist und zu den neutrophilen Dermatosen gezählt wird.
Prädilektionsstelle ist in 80% der Fälle der Unterschenkel. Die Ätiopathogenese ist
noch nicht
genau geklärt, angenommen wird eine destruktive autoinflammatorische Reaktion des
Immunsystems. Zudem gilt das Pathergiephänomen als ein möglicher Auslöser. Einen etablierten
Therapiestandard gibt es bisher nicht. Wir stellen einen Patientenfall mit ungewöhnlicher
Lokalisation eines Pyoderma gangraenosum am Rücken vor und Beleuchten die Komplexität
der
Therapie. Nach Therapieversagen mit hochdosierten Glukokortikosteroiden und dem
TNF-α-Inhibitor Adalimumab zeigte letztlich eine Kombinationstherapie mit intravenösen
Immunglobulinen und dem IL-12/23-Antikörper Ustekinumab ein erfolgreiches Ansprechen.
Abstract
Pyoderma gangraenosum (PG) is a rare disease characterized by a sterile, painful,
and rapidly progressive ulceration with a lividly colored and undermined rim and is
considered a neutrophilic dermatosis. The predilection site is the lower leg in 80%
of cases. The etiopathogenesis has not yet been precisely clarified, but a destructive
autoinflammatory reaction of the immune system is assumed. Also it is known that the
pathergy phenomenon is an important trigger. To date, there is no established standard
of treatment; systemic immunosuppression is usually required. We present an impressive
patient case with unusual localization of pyoderma gangraenosum on the back and we
highlight the complexity of the therapy. After treatment failure of high-dose intravenous
corticosteroid therapy and TNF-α-Inhibitor Adalimumab a combination therapy using
intravenous immunoglobulins and IL-12/23-antibody ustekinumab ultimately showed good
results.
