Inborn Errors of Immunity und rheumatisch-entzündliche Autoimmunität

Zusammenfassung

Rheumatische Symptome wie Arthritis, Vaskulitis und Autoinflammation können (Erst)-Manifestation von IEI sein und sind meist assoziiert mit pathologischen Infektionen und weiteren autoimmun-autoinflammatorischen Komplikationen. Das Erkennen eines IEI hat entscheidende therapeutische Implikationen und ermöglicht teilweise gezielte Ansätze, die die zugrunde liegende Immunpathologie adressieren. Eine frühe Diagnosestellung soll systemische Komplikationen und Organschäden vermeiden und die Lebensqualität der Betroffenen verbessern. Gen-Varianten, die mit seltenen monogenen IEI assoziiert sind, findet man aber auch in Verbindung mit polygenen rheumatischen Erkrankungen. Einblicke in Kandidatengene und ein besseres Verständnis genetischer, molekularer sowie immunologischer Mechanismen sowie genetischer Netzwerke und epigenetischer und weiterer Einflussfaktoren auf diese Mechanismen und Immuneffektoren können helfen, zielgerichtete Therapien auch für polygene entzündlich-rheumatische Krankheitsbilder zu finden.
