Arthralgien bei Psoriasis vulgaris – manchmal lohnt ein zweiter Blick

Zusammenfassung

Anamnese Die 71-jährige Patientin berichtete von langjährig bestehenden entzündlichen Arthralgien bei Psoriasis vulgaris. Eine Therapie mit Methotrexat hatte zu Leberwerterhöhungen geführt, unter Behandlung mit dem TNFα-Inhibitor Adalimumab persistierten die Beschwerden.

Untersuchungen Mittels Arthrosonografie und MRT konnte ein vorwiegend periartikuläres Inflammationsmuster an Handgelenken, Metakarpophalangeal-Gelenken und proximalen Interphalangeal-Gelenken ohne postentzündliche Stigmata einer Psoriasisarthritis objektiviert werden. Laborchemisch fand sich ein erhöhter anti-HCV-Titer mit hoher Viruslast im Sinne einer floriden Hepatitis C.

Diagnose Eine vorwiegend extrahepatisch manifestierte Hepatitis C mit assoziierter Periarthritis wurde diagnostiziert.

Therapie und Verlauf Unter kombinierter, interferonfreier antiviraler Therapie sistierten die Gelenkbeschwerden.

Folgerung Die Hepatitis C kann sich durch extrahepatische Manifestationen im Sinne einer periartikulären Inflammation äußern. Entzündlich suspekte Arthralgien bei Psoriasis vulgaris sind nicht gleichbedeutend mit der Diagnose einer Psoriasisarthritis und sollten fachgerecht rheumatologisch evaluiert werden.

Abstract

History and clinical findings The 71-year-old patient reported long-standing inflammatory arthralgia due to psoriasis vulgaris. Therapy with methotrexate led to elevated liver values; the symptoms persisted under treatment with the TNFα inhibitor adalimumab.

Medical examinations Using arthrosonography and MRI, a predominantly periarticular inflammatory pattern on the wrists, metacarpophalangeal joints and proximal interphalangeal joints could be objectified without post-inflammatory stigmata of psoriatic arthritis. Laboratory diagnostics revealed an elevated anti-HCV titer with a high viral load consistent with a florid hepatitis C.

Diagnosis A predominantly extrahepatic hepatitis C with associated periarthritis was diagnosed.

Therapy and course With combined, interferon-free antiviral therapy, arthralgia suspended completely.

Conclusion Hepatitis C can appear through extrahepatic manifestations in the form of periarticular inflammation. Suspicious inflammatory arthralgias in psoriasis vulgaris are not conterminous with the diagnosis of psoriatic arthritis and should be evaluated by a professional rheumatologist.

  • Bei dem Verdacht auf eine entzündliche Gelenkerkrankung sollte eine fachgerechte rheumatologische Abklärung angestrebt werden, um mögliche Differenzialdiagnosen umfangreich zu evaluieren.

  • Entzündlich suspekte Arthralgien bei Psoriasis vulgaris sind nicht gleichbedeutend mit der Diagnose einer Psoriasisarthritis und sollten ebenfalls fachgerecht rheumatologisch abgeklärt werden.

  • Eine Hepatitis C kann sich klinisch durch extrahepatische Manifestationen äußern, eine nicht erosiv verlaufende (Peri-)Arthritis ist möglich. In der MRT-Bildgebung kann ein vorwiegend periartikuläres Inflammationsmuster nachvollzogen werden.

  • Bei extrahepatisch manifestierter Hepatitis C mit immunologischen Phänomenen steht die antivirale Therapie im Vordergrund.
