Off the expert office: Assessment and differentiation of Ménière’s disease and vestibular migraine
The diagnostic criteria for Ménière’s disease and vestibular migraine show an overlap in clinical practice. Thus, it can be difficult to differentiate with a high diagnostic probability between these two disorders. The combination of accompanying symptoms should guide diagnostic reasoning since there is no single specific symptom for one of the two entities. The symptom pattern of a higher degree of hearing impairment, an impairment of saccular function and semicircular canal function (hVOR disorder during thermal testing, i. e. in the low-frequency range) can objectively be measured and indicate rather for the presence of Meniere’s disease then for vestibular migraine. Here, we propose a new table for the assessment and appraisal of Ménière’s disease, which takes into account duration as well as severity of attacks and evaluates functional disability by objective vestibulo-cochlear findings.
