Syndesmosis Injuries
Syndesmosenverletzungen entstehen bei Sprunggelenkfrakturen oder rein ligamentär. können stabil oder instabil sein. In den letzten Jahren konnten zunehmende Erkenntnisse über den Verletzungsmechanismus und die Therapie gewonnen werden, kontroverse Diskussionen werden über Diagnostik, neue operative Verfahren wie Suture-Button-Systeme und die Nachbehandlung geführt. Eine zunehmende Bedeutung erlangten die knöchernen Ausrisse der vorderen und hinteren Syndesmose, dessen Refixation großzügig erfolgen sollte. Bei chronischer Instabilität ist eine Ursachenanalyse entscheidend, operative Verfahren beinhalten dynamische Verfahren, Sehnenplastiken oder knöcherne Rekonstruktionen. Entscheidend für das Outcome ist bei allen Verfahren die anatomische Rekonstruktion der Sprunggelenkgabel.
