Der Stellenwert des Hip-Spine-Syndroms in der Hüftendoprothetik: Einfluss auf den Behandlungserfolg und therapeutische Konsequenzen

Ein korrektes Zusammenspiel von Wirbelsäule, Becken und Hüfte ist eine wesentliche Voraussetzung für einen komplikationslosen Verlauf nach Implantation einer Hüfttotalendoprothese. Wirbelsäulenpathologien wie Degeneration, Frakturen oder spinopelvine Dysblance mit und ohne lumbale Fusionen gehen eng mit einer erhöhten Gefahr eines Impingements oder sogar einer Luxation der Prothese einher. Um dieses Risiko entscheidend reduzieren zu können, sind verschiedene Parameter zur Quantifizierung der Risikogruppen erforderlich. So sind die Kenntnis des Vorliegens einer Steifheit der Wirbelsäule (Veränderung der Beckenkippung zwischen Stehen und Sitzen von < 10°) sowie einer sagittalen Wirbelsäulendeformität (PI-LL-Mismatch > 10 bzw. 20°) essenziell, um die entsprechenden Risikopatienten erkennen zu können. Durch eine gezielte Anamnese und Untersuchung kann das individuelle Risikoprofil eingeschätzt werden. Vor Implantation einer Hüfttotalendoprothese empfiehlt sich bei diesen Risikopatienten ein routinemäßiges präoperatives Work-up. Durch die Informationen aus standardisierten präoperativen Röntgenaufnahmen im Sitzen und Stehen (Becken a. p. liegend und stehend, Wirbelsäule und Becken lateral stehend und sitzend) kann intraoperativ dementsprechend reagiert werden: Bei einer Steifheit der Wirbelsäule sollte zur Verminderung des Luxationsrisikos auf eine Pfannenpositionierung mit erhöhter Anteversion und auf ein ausreichendes Offset sowie auf einen luxationssichereren größeren Kopf geachtet werden. Bei einer sagittalen Wirbelsäulendeformität muss vor allem der funktionellen koronaren Beckenebene Beachtung geschenkt werden, wobei der großzügige Einsatz von Double-Mobility-Pfannen nicht vernachlässigt werden sollte. Digitale Systeme wie Navigation und Robotik können die Komponentenpositionierung idealisieren, wobei bisher kaum Evidenz für eine reduzierte Komplikationsrate besteht und weitere Studien erwartet werden.
