Multiresistente Erreger in der stationären Palliativmedizin
Hygienemaßnahmen und AntibiotikamanagementPatient*innen in der Palliativversorgung haben ein hohes Risiko für die Kolonisation mit MRE, daher erscheint es sinnvoll, bei stationärer Aufnahme ein generelles Screening durchzuführen. Der Beitrag thematisiert den Einsatz von Antibiotika und die resultierenden psychischen und physischen Belastungen bei den Betroffenen.

Patient*innen in der Palliativversorgung haben ein hohes Risiko für die Kolonisation mit MRE. Neben den häufigen Kontakten zum Gesundheitssystem tragen sicher auch die hohen Verordnungsraten von Antibiotika in der letzten Lebensphase zu den hohen MRE-Raten bei.

Ein MRE-Nachweis führt regelhaft zur Isolierung innerhalb des Krankenhauses, was wiederum eine eingeschränkte Palliativversorgung sowie psychische Schäden bei den Betroffenen zur Folge hat.

Individuelle Hygienekonzepte innerhalb der Palliativversorgung können helfen, das o. g. Ziel der Palliativversorgung auch für MRE-positive Patient*innen zu ermöglichen, ohne den Schutz der Mitpatient*innen zu vernachlässigen.

Patient*innen und ihre Angehörigen sollten in den Entscheidungsprozess hinsichtlich der Antibiotikaeinnahme sowie der Hygiene- und Isolierungsmaßnahmen einbezogen werden.

Weitere Forschung zu Vor- und Nachteilen einer Antibiotikagabe sind notwendig, um palliativmedizinisch betreute Patient*innen vor unnötigen Kollateralschäden durch die Behandlung zu schützen.

In der stationären wie ambulanten Palliativversorgung sollte ABS-Expertise zur Anwendung kommen.

Neben der Basishygiene wurden bereits Hygienekonzepte entwickelt, die auf drei Elementen beruhen: MRE-Screening (bereits vor der Aufnahme), Barrierepflege und Monitoring.
