Zusammenfassung

Akkommodation ist die natürliche Zu- oder Abnahme der Refraktion des Auges bei Änderung des Beobachtungsabstands. Sie beruht auf der Verformbarkeit der natürlichen Augenlinse. Durch Untersuchung der Akkommodation und der an sie gekoppelten Konvergenz lassen sich definierte Krankheitsbilder diagnostizieren und von neuroophthalmologischen Erkrankungen abgrenzen. Überflüssige diagnostische Maßnahmen werden so vermieden, Patientenängste reduziert.

Abstract

Accommodation is the natural increase or decrease of the refraction of the eye lens as a function of the fixation distance. It relies on the proper elasticity of the natural eye lens. The evaluation of accommodation and the coupled convergent eye movements enables us to make the diagnosis of certain eye-related conditions and allows the differential diagnostic from some of the “neuroophthalmic diseases”. This may avoid some unnecessary diagnostic procedures and reduce patient anxiety in terms of having a health threatening disease.

  • Akkommodation beruht auf der Fähigkeit der natürlichen Augenlinse, ihre Form und damit ihren Brechindex zu verändern. Die Theorie von Helmholtz zum Mechanismus der Akkommodation ist damit nach wie vor gültig.

  • Störungen der Akkommodation können zu asthenopischen Beschwerden führen und werden häufig unterdiagnostiziert.

  • Zu den wichtigsten Untersuchungsverfahren der Akkommodation zählen die Annäherungsmethoden, das Vorschalten von Minusgläsern und die dynamische Skiaskopie.

  • Bei einer brillenkorrigierten höheren Ametropie unterscheiden sich Akkommodationsaufwand und Akkommodationserfolg. Dies bewirkt einen im Vergleich zur Emmetropie früheren (späteren) Beginn der Presbyopie bei Hyperopie (Myopie) im Vergleich zu einem nicht fehlsichtigen Auge.

  • Eine Störung der Akkommodation kann sich klinisch durch einen Konvergenzexzess manifestieren.

  • Sowohl bei hypo- als auch bei normakkommodativen Konvergenzexzess besteht die Therapie in der Verordnung einer Nahaddition.

  • Isolierte Störungen der Akkommodation wie ein Akkommodationsspasmus oder ein isolierter Akkommodationsverlust haben selten eine fassbare neurologische Ursache.

  • Störungen der Akkommodation sind einer wirksamen Behandlung zugänglich.

  • Nach jetziger Studienlage kann der Akkommodationsvorgang an sich nicht für die Myopieentstehung oder die Myopieprogression verantwortlich gemacht werden.
