Notfallmedizinisches Vorgehen beim Ertrinkungsunfall
Prehospital Emergency Care for DrowningErtrinkungsunfälle stellen im Rettungsdienst eine der selteneren Einsatzindikationen dar. Aufgrund der verschiedenen Gewässertypen (Hochsee, Küstenmeer, Badeseen, Flüsse, Schwimmbäder, private Gewässer) sind diese Einsätze entsprechend ihrer Rahmenbedingungen teilweise sehr anspruchsvoll im Hinblick auf die Rettung und notfallmedizinische Versorgung der Betroffenen. Dieser Artikel gibt einen Überblick zur Pathophysiologie, Rettung und präklinischen notfallmedizinischen Versorgung von Ertrinkungsunfallopfern.
Abstract
Drowning accidents are one of the less common indications for emergency services. Due to the different types of water (open sea, coastal sea, bathing lakes, rivers, swimming pools, private bodies of water), these missions are sometimes very demanding in terms of rescue and emergency medical care for those affected. In the event of cardiovascular arrest, which is usually the result of hypoxia in the event of a drowning accident, five ventilation strokes are recommended to start resuscitation. When performing resuscitation after a drowning accident, the factors “feasibility”, “chances of survival” and “risks for the rescue personnel” should be part of the further decision-making process. Therefore, submersion time is an essential factor for the treatment result, which is why it should be determined as precisely as possible. Patients with additional accidental hypothermia should be admitted to a center with the possibility of extracorporeal rewarming.

Pathophysiologisch im Vordergrund stehen bei einem Ertrinkungsunfall die Hypoxie und alle damit einhergehenden Folgen.

Bei der Rettung aus dem Gewässer ist die Eigensicherung der Rettungskräfte von elementarer Bedeutung.

Die Submersionszeit stellt einen wesentlichen Faktor für das Behandlungsergebnis dar, weswegen sie so genau wie möglich ermittelt werden sollte.

Die präklinische Erstversorgung beim Ertrinkungsunfall folgt den Prinzipien des ABCDE-Schemas.

Für den Beginn einer Reanimation nach Ertrinkungsunfall werden vom ERC 5 initiale Beatmungshübe empfohlen.

Sofern sichere Todeszeichen ausgeschlossen werden können, sollten bei einem Patienten mit begleitender akzidenteller Hypothermie die Reanimationsmaßnahmen bis zur Übergabe in einer Klinik mit der Möglichkeit zur extrakorporalen Wiedererwärmung fortgesetzt werden.
