Akute tubulointerstitielle Nephritis bei Kindern und Jugendlichen

ZUSAMMENFASSUNG

Die akute tubulointerstitiellen Nephritis (TIN) stellt eine wichtige Ursache des akuten Nierenversagens bei Erwachsenen und Kindern dar. Auslöser der TIN sind eine Vielzahl von Medikamenten, systemische Erkrankungen und Infektionen. Weiterhin existieren idiopathische Fälle ohne identifizierbare Ursache. Eine Sonderform der Erkrankung ist die tubulointerstitielle Nephritis mit Uveitis (TINU), die bei Kindern und Jugendlichen häufiger als bei Erwachsenen vorkommt. Klinische Symptomatik und Laborbefunde der TIN sind häufig unspezifisch, sodass die definitive Diagnose nur bioptisch gesichert werden kann. Die Entfernung der auslösenden Substanz bzw. die Behandlung der Grunderkrankung sind essenziell für die Therapie der TIN. Der Nutzen der Glukokortikoidtherapie wurde bisher nicht durch randomisierte, kontrollierte Studien belegt.

ABSTRACT

Acute tubulointerstitial nephritis (TIN) is an important cause of acute renal failure in adults and children. TIN can be triggered by numerous medications, systemic diseases and infections. Furthermore, idiopathic cases without identifiable cause exist. A special form of the disease is tubulointerstitial nephritis with uveitis (TINU), which is more frequent in children and adolescents than in adults. Clinical symptoms and laboratory findings of TIN are often unspecific, so that the definitive diagnosis can only be made by a biopsy. Removal of the triggering substance or treatment of the underlying disease is essential for the therapy of TIN. The benefit of glucocorticoid therapy has not yet been proven by randomized, controlled trials.
