Dieser Beitrag soll den aktuellen Stand in Diagnostik und Therapie bei gutartigen Erkrankungen des Magens und Duodenums komprimiert darstellen. Dabei handelt es sich – wie kaum anderenorts – um eine Schnittstelle in der Medizin, in der konservative gastroenterologische Therapieansätze, endoskopische Interventionsmöglichkeiten und zumeist schonende minimalinvasive Operationsmethoden unterschiedlichste Behandlungsmöglichkeiten eröffnen.

Kernaussagen
  • Dieser Beitrag stellt die Diagnostik und Therapie bei gutartigen Erkrankungen des Magens und Duodenums (mit Ausnahme von gastrointestinalen Stromatumoren und neuroendokrinen Tumoren) komprimiert dar.

  • Konservative gastroenterologische Therapieansätze, endoskopische Interventionsmöglichkeiten und zumeist schonende minimalinvasive Operationsmethoden bieten unterschiedlichste Behandlungsmöglichkeiten dieser Erkrankungen.

  • Gastritiden werden eingeteilt in die akute und chronische Form, wobei Erstere i. d. R. selbstlimitierend verläuft, während Letztere ggf. eine Helicobacter-pylori-Eradikation erfordert.

  • Die funktionelle Dyspepsie ist neben dem oft gleichzeitig auftretenden Reizdarmsyndrom die häufigste funktionelle Erkrankung des Gastrointestinaltrakts; auch hier ist eine Helicobacter-pylori-Diagnostik und ggf. -Eradikation indiziert.

  • Peptische Magen- und Duodenalulzera imponieren durch mehr oder weniger ausgeprägte Ulkusblutungen und stellen die häufigste gastrointestinale Blutungsursache dar. In der endoskopischen Therapie wird – befundabhängig – die duale Blutungskontrolle angestrebt.

  • Gutartige oder Borderline-Magen- und Duodenaltumoren zeichnen sich durch eine gewisse Heterogenität, ein eher seltenes Auftreten und eine zumeist uncharakteristische Symptomatik aus. Entsprechend vielfältig gestaltet sich die Therapie.

  • Bei Ulkuskomplikationen und gutartigen Tumoren an Magen und Duodenum kommt ein breites Spektrum an möglichen Operationen in Frage, wobei nahezu alle Eingriffe alternativ in offener oder laparoskopischer/robotischer Technik durchgeführt werden können.
