Nichterkennung einer Skabies trotz positiver Familienanamnese

Zusammenfassung

Bei einer Patientin wurde von einem Dermatologen trotz mehrfacher Konsultationen eine Skabies nicht diagnostiziert, obgleich die Tochter der Patientin von eben diesem Dermatologen zeitnah unter der Diagnose einer Skabies behandelt worden war. Erst nachdem die Patientin mit verschlechtertem Hautzustand in einer Hautklinik stationär aufgenommen wurde, erfolgte die korrekte Diagnosestellung und Therapie. Die Schlichtungsstelle bewertete die Nichtberücksichtigung der positiven Familienanamnese für Skabies als vorwerfbaren Diagnosefehler.

Bei pruriginösen Erkrankungen unklarer Genese sollte stets auch an eine Skabies gedacht und eine Familienanamnese erhoben werden. Falls diese positiv ist, sind die Familienangehörigen als Kontaktpersonen, d. h. Personen mit längerem Kontakt zu Patienten mit gewöhnlicher Skabies oder Personen auch mit nur kurzem Kontakt zu Patienten mit Scabies crustosa, zu behandeln. Diese erhalten zumindest eine einmalige Sicherheitsbehandlung mit Permethrin, dem aktuellen Behandlungsstandard.

Abstract

A patient was not diagnosed with scabies by a dermatologist despite several consultations, although the patientʼs daughter was treated by the same dermatologist at the same time under the diagnosis of scabies. Only after the patient was admitted to a Department of Dermatology because of a worsened skin condition, the correct diagnosis was made and therapy was started. The Independent Medical Expert Council (IMEC) assessed the non-consideration of the positive family history for scabies as an accusable diagnostic error.

In the case of pruriginous diseases of unclear genesis, scabies should always be considered and a family history should be taken. If this is positive, family members should be treated as contact persons, i. e. persons with longer contact to patients with common scabies or persons with short contact to patients with scabies crustosa. These persons should receive at least one safety treatment with permethrin, the current standard for the management of scabies.
