Die Bedeutung von motorischen Verhaltensänderungen durch Insuffizienzhinken für die Belastung und Beanspruchung von Gelenken

The importance of motor behavioural changes due to insufficiency-induced limping respecting stress and strain on joints

Zusammenfassung

Insuffizienzhinken nach degenerativen Veränderungen oder Verletzung der großen Beingelenke ist eine typische Änderung des Bewegungsverhaltens. Es ist eine motorische Verhaltensänderung, die die Schonung des betroffenen Gelenks und der geschwächten Muskulatur zum Ziel hat. So werden beim Gehen Körperpositionen eingenommen, durch die betroffenen Gelenke geringer, jedoch andere stärker belastet werden. Eine reduzierte Belastung bedeutet jedoch nicht zwingend auch eine verminderte Beanspruchung der Gelenke! Während die Belastung, die beim Gehen im Sinne von kompressiven Kräften auf das Hüftgelenk wirkt, durch die Hinkmuster reduziert wird, kann die Beanspruchung der gelenkigen Strukturen sehr ungünstige Formen annehmen. Es wird im vorliegenden Artikel klargestellt, dass nicht die Belastung das Gewebe der Gelenke unmittelbar beeinflusst, denn es sind vielmehr die Verteilung des Drucks auf der Gelenkoberfläche und die mechanische Spannung im Knorpel sowie im unterliegenden Knochen. Deswegen ist in der Therapie eine zielgerichtete Interventionsstrategie zu wählen, bei der die aus der Belastung resultierende Beanspruchung berücksichtigt wird. Aus Sicht der Autoren müssen sich Patienten mit manifestierten Insuffizienzhinkmunstern im Rahmen der Sekundärprävention zuerst einer Bewegungsschulung unterziehen, sonst können sich Spätschäden einstellen und eine nachhaltige Bindung an einen „bewegten Alltag“ nachhaltig stören. Die Betreuung durch entsprechend geschulte Sport- und Bewegungstherapeuten eignet sich hierfür im hohen Maße.

Summary

Insufficiency-induced limping after degenerative changes or injury to the leg joints is a typical change in movement behaviour. It is a motor behavioural change that seeks to protect the affected joint and weakened muscles. Due to the different body positions encountered when walking, the affected leg-joints are subjected to different degrees of stress. However, a reduced load does not necessarily mean a reduced strain on the joints. While the strain on the hip joint when walking in the sense of compressing forces is reduced by the hinge patterns, the stress of the articulated structures can take very unfavourable forms.

This article emphasises the fact that it is not the load that directly affects the tissue of the joints, but rather the distribution of pressure on the joint surface and the mechanical tension in the cartilage and in the underlying bone. Therefore, a targeted intervention strategy must be chosen in therapy, taking into account the stress resulting from the strain. From the authors’ point of view, patients with manifested insufficiency-induced limping patterns must first undergo exercise training as part of secondary prevention. Otherwise, damage can occur later on and impede sustainable adherence to a physically active lifestyle. The treatment provided by appropriately trained sports and exercise therapists is highly suitable in such a case.
