Kognitive Remediation und soziales Kompetenztraining bei Schizophrenie

Bei der Behandlung schizophrener Erkrankungen hat die Verbesserung der psychosozialen Funktionsfähigkeit eine zentrale Bedeutung. Die aktualisierte deutsche S3-Praxisleitlinie Schizophrenie empfiehlt dafür kognitive Remediation und soziales Kompetenztraining. Beide Verfahren haben sich als effektiv erwiesen. Die kombinierte Anwendung beider Verfahren ist dabei besonders erfolgversprechend.
Kernaussagen

  • Schizophrene Erkrankungen gehen mit deutlichen psychosozialen Beeinträchtigungen einher, die trotz Symptomremission bestehen und die Betroffenen sehr belasten und im Alltag einschränken.

  • Die (Wieder-)Erlangung eines zufriedenstellenden psychosozialen Funktionsniveaus ist ein wesentliches Therapieziel bei schizophrenen Erkrankungen.

  • Diesbezügliche Therapiemaßnahmen setzen in der Regel an wesentlichen Einflussfaktoren der psychosozialen Funktionsfähigkeit an, wie der (sozial-)kognitiven Leistungsfähigkeit, sozialen Kompetenzen und der Negativsymptomatik.

  • Kognitive Remediationsprogramme sind wirksame Therapieoptionen, die sowohl die jeweils anvisierten kognitiven Zielfunktionen als auch das psychosoziale Funktionsniveau verbessern.

  • Soziale Kompetenztrainings können soziale Fertigkeiten verbessern, Negativsymptomatik vermindern und die psychosoziale Funktionsfähigkeit unterstützen.

  • Die aktuelle deutsche Behandlungsleitlinie Schizophrenie empfiehlt kognitive Remediation und soziales Kompetenztraining zur Behandlung schizophrener Psychosen.

  • Weiterentwicklungen zu einer noch stärkeren Verbesserung der psychosozialen Funktionsfähigkeit schizophren Kranker zielen auf eine integrierte Anwendung beider Therapieansätze.
