Lösungsmöglichkeiten bei fehlgeschlagenen Osteosynthesen in der Mund-, Kiefer- und Gesichtschirurgie
Approaches in Failed Osteosynthesis in Oral and Maxillofacial SurgeryZusammenfassung
Die osteosynthetische Versorgung mit Miniplatten oder Rekonstruktionsplatten stellt in der Mund-, Kiefer- und Gesichtschirurgie den operativen Standard in Versorgung von Gesichtsschädelfrakturen und bei der Fixation autologen Knochentransplantate dar. Intra- und extraorale Zugangswege zum knöchernen Gesichtsschädel sowie die Belastung durch Kaukräfte, Lagebeziehung zur Mundhöhle und Nasennebenhöhlensystem sowie die Okklusion können zu einem Misserfolg der Osteosynthese beitragen. Zu den häufigsten Komplikationen gehören Materiallockerung, Materialbruch, sekundäre Fragmentdislokation und Infektion.
Die Plattenosteosynthese hat sich in der Mund-, Kiefer- und Gesichtschirurgie für die Frakturversorgung als Standard etabliert. Die anatomisch-ästhetischen Einschränkungen der extraoralen und intraoralen Zugangswege im Gesichtsbereich haben einen Einfluss auf die Frakturversorgung. Die unmittelbare Belastung durch Kaukräfte, die anatomische Lagebeziehung zur Mundhöhle und zum Nasennebenhöhlensystem und die Einbeziehung der Okklusion sind Faktoren, die zu einem Misserfolg der Osteosynthese führen können.
Abstract
Osteosynthesis using mini and reconstruction plates represent the state of the art in the operative treatment of fractures and fixation of osseous transplants in oral and maxillofacial surgery. Intra- and extraoral approaches to the facial skeleton as well as mastication forces, the relationship to the oral cavity and the sinus system, and the occlusion could add up to a failure of osteosynthesis. Loosening of the osteosynthetic material, breakage of the material, secondary dislocation of the fragments, and infections represent the most common complications.
