Die aus dem englischen Sprachgebrauch etablierte Bezeichnung „Wide Awake Local Anesthesia no Tourniquet“, abgekürzt WALANT, beschreibt eine Technik der lokalen Tumeszenzanästhesie. Durch die Applikation eines Lokalanästhetikums in Kombination mit einem Vasokonstriktor können grundsätzlich fast alle Operationen an Hand und Unterarm in Schmerzfreiheit und ohne Blutsperre durchgeführt werden. Dabei spielt die richtige Patientenselektion unter Berücksichtigung der Kontraindikationen eine wichtige Rolle.

Kernaussagen
  • Die WALANT-Technik gewinnt weltweit zunehmend an Popularität, da es sich um ein günstiges, einfach zu applizierendes und umweltfreundliches Verfahren mit vielen Vorteilen gegenüber anderen Anästhesieformen für Arzt und Patient handelt.

  • Auch wenn es sich um ein insgesamt risikoarmes Verfahren handelt, spielt die richtige Patientenselektion unter Berücksichtigung der Kontraindikationen eine wichtige Rolle.

  • In mehreren prospektiven Studien sowie Multicenterstudien mit großen Fallzahlen konnte kein erhöhtes Risiko von Fingernekrosen bei einer sachgemäßen Injektion von Lidocain mit niedrigdosierten Epinephrin festgestellt werden.

  • Dennoch sollte aufgrund vereinzelter Fallberichte der jüngsten Zeit über aufgetretene Fingernekrosen nach Applikation von Lidocain und Epinephrin das Verfahren mit entsprechendem Sachverstand angewandt und Phentolamin als Antidot vorgehalten werden.

  • Kontraindikationen zur WALANT-Technik bestehen bei schweren flächigen Vernarbungen der Hohlhand, vorbestehenden Perfusionsstörungen und bei Patienten, die einer Operation in Lokalanästhesie grundsätzlich ablehnend oder zurückhaltend gegenüber stehen.
