Akrolentiginöses Melanom unter der (Fehl-)Diagnose einer Onychomykose

Zusammenfassung
Ein Patient wurde unter der Diagnose einer Paronychie und Onychomykose des Mittelfingers rechts ohne mykologischen Pilznachweis langfristig topisch und systemisch antimykotisch behandelt. Bei Therapieresistenz erfolgte nach Vorstellung bei einem anderen Hautarzt eine Hautbiopsie, die ein akrolentiginöses malignes Melanom ergab, das unter Teilamputation des Fingers operativ versorgt werden musste. Nach Facharztstandard war die Verzögerung der letztlich zielführenden Diagnostik als Behandlungsfehler zu bewerten.
Unklare entzündliche Veränderungen oder Tumoren an den Akren sollten immer zur Differenzialdiagnose eines akrolentiginösen malignen Melanoms veranlassen, auch wenn prima vista viel häufigere Erkrankungen wie eine Onychomykose oder Verrucae vulgares wahrscheinlicher sind. Spätestens bei Nichtansprechen einer eingeleiteten Therapie sollte die Diagnose kritisch überprüft und ggf. eine dermatohistologische Befundsicherung durchgeführt werden.
Abstract
A patient was diagnosed with paronychia and onychomycosis of the right middle finger and was treated with antifungals topically and systemically for a prolonged period although no dermatophytes had been identified neither by microscopy nor by culture. Since therapy was unsuccessful, the patient consulted another dermatologist who performed a skin biopsy which revealed an acrolentiginous malignant melanoma. Surgical treatment with partial amputation of the finger was performed. According to the medical specialist standard, the diagnostic delay was assessed to be a treatment error.
Unclear inflammatory changes or tumors at the fingers or toes should always include the differential diagnosis of acrolentiginous malignant melanoma, even if prima vista much more frequent diseases such as onychomycosis or verrucae vulgar may be more likely. At the latest in case of a non-response to an initiated therapy, the diagnosis should be critically reconsidered and, if necessary, a skin biopsy with dermatohistological workup should be carried out.
