Medikamentöse Therapie der ADHS bei Erwachsenen

Die pharmakologische Behandlung der Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) ist bei Kindern, Jugendlichen und Erwachsenen der zentrale Baustein eines therapeutischen Gesamtkonzeptes, in dem auch psychosoziale Interventionen mit Psychoedukation oder spezifische psychotherapeutischen Maßnahmen, wie die kognitive Verhaltenstherapie, ihren Platz haben.

Kernaussagen
  • Die pharmakologische Therapie stellt ein zentrales Behandlungselement der ADHS im Erwachsenenalter dar und sollte auf der Grundlage einer informierten und partizipativen Therapieentscheidung durch den Patienten und unter Einbeziehung psychosozialer Behandlungsangebote erfolgen.

  • Die Wirksamkeit einer pharmakologischen Behandlung von ADHS im Erwachsenenalter ist durch Metaanalysen kontrollierter Behandlungsstudien gut belegt.

  • Behandlungseffekte zeigen sich sowohl im Sinne der Reduktion der Kernsymptome der ADHS sowie der im Erwachsenenalter oft assoziierten emotionalen Dysregulation, als auch in der Verbesserung der Alltagsfunktionen und Lebensqualität.

  • Als Mittel der 1. Wahl stehen für die Pharmakotherapie der ADHS im Erwachsenenalter die Psychostimulanzien Methylphenidat und Lisdexamphetamin zur Verfügung; daneben ist auch Atomoxetin für die Behandlung bei Erwachsenen zugelassen, das nicht dem Betäubungsmittelrecht unterliegt.

  • Bei der medikamentösen Behandlung der ADHS im Erwachsenenalter sollten stets mögliche unerwünschte Nebenwirkungen, insbesondere kardiovaskuläre Effekte, im Blick behalten werden.
