Zusammenfassung
Trotz verschiedenster reformpolitischer Änderungen und Schaffung ausreichender rechtlicher
Voraussetzungen konnte sich in Deutschland bisher keine sektorendurchlässige personale
und durchgehend verzahnte Patientenversorgung mit sinnvoller ökonomischer Nutzung
gemeinsamer Apparate und personeller Ressourcen etablieren. Zum funktionierenden und
nachhaltigen Aufbau einer verzahnten Patientenversorgung gehört der Ausbau kooperativer,
fachverbindender und interdisziplinärer Praxisstrukturen und Öffnung für neue Strukturen,
auch unter Zugrundelegung des neuen Partnerschaftsgesellschaftsgesetzes sowie auf
Krankenhausseite die Etablierung einer sektorenübergreifenden Patientenversorgung,
bei dem freiberuflich tätige Ärzte aus dem ambulanten Sektor mit Ärzten aus dem Kliniksektor
zusammen arbeiten und beide Seiten ambulant als auch stationär tätig sind.
Abstract
In contrast to most European countries the German medical system is departed in in-hospital
vs. outpatient care, offered by free-lancing ambulant specialists on one side and
specialists in hospitals on the other side leading to costly and unneeded double diagnostics,
not using apparative and personal resources together and therefore to be more inefficient
and less economic. Although outpatient specialist are responsible for all emergency
case, it is already nowadays reality, that hospitals provide a substantial proportion
of outpatient emergency care without having the structure (e.g. physicians on call
are frequently non-specialists and therefore more expensive in providing emergency
services in hospitals). Although stimulated by numerous governmental legal modifications
interconnected in-hospital/outpatient medical care did not increase sufficiently since
20 years in Germany.
