ZUSAMMENFASSUNG

Das metabolische Syndrom ist in aller Regel mit einer Insulinresistenz und einer viszeralen Fettverteilung assoziiert, die bei der Entstehung klinischer Facetten des metabolischen Syndroms, zum Beispiel der Erhöhung des arteriellen Blutdrucks und der Dyslipidämie, eine entscheidende Rolle spielen. In Bezug auf das kardiovaskuläre System gibt es zunehmend Hinweise, dass eine Insulinresistenz und ektope Lipidablagerung am Herzen, insbesondere das epikardiale Fett, mit Inflammation, Fibrosierung und eingeschränkter diastolischer Funktion bzw. Herzinsuffizienz assoziiert sein können. Die klinische Prognose von Patienten mit metabolischem Syndrom und Herzinsuffizienz ist sehr schlecht, und daher könnten diese neuen Zusammenhänge zwischen Adipositas, Insulinresistenz, Fettverteilung, Inflammation und Herzfunktion künftig eine Rolle bei einer differenzierten kardiometabolischen Früherkennung und Therapie von Risikoindividuen sein.
