Zusammenfassung

Bei Tibiakopffrakturen handelt es sich meist um schwere knöcherne Verletzungen, die einer operativen Therapie bedürfen. Diese stellt aufgrund der Komplexität der Fraktur einen hohen Anspruch an den Operateur, da nur nach sorgfältiger Analyse der Fraktur, Wahl des korrekten, weichteilschonenden Zugangs und Verwendung eines passenden Implantats eine optimale Reposition und Retention mit Wiederherstellung der Gelenkkongruenz erzielt werden kann. Dieser Beitrag stellt die modernen Klassifikationen und Zugangswege vor.

Abstract

Complex fractures of the proximal tibia require a thorough analysis of the fracture type based on computer tomography. Initial diagnostics have to rule out a compartment syndrome, injury of nerves and vessels as well as other concomitant injuries. Definitive treatment is performed after an interval of 4 – 10 days often depending on the soft tissue status, which has to be included in all later decisions. A variety of operative approaches and implants is available for definitive stabilization.
