Akute Pankreatitis – Teil 2: Therapeutisches Vorgehen

In der Therapie der akuten Pankreatitis wird eine schrittweise Eskalation der Therapie (Step-up Approach) angestrebt: Von der konservativen Behandlung über die gastroskopische oder perkutane Drainageeinlage zur minimalinvasiven Nekrosektomie und schließlich zur offenen Nekrosektomie, wenn andere Verfahren versagen. Durch dieses Vorgehen konnte die Rate an schweren Komplikationen und das Auftreten von Spätfolgen deutlich gesenkt werden.

Kernaussagen
  • Die Therapie der Erkrankung hat sich über die vergangenen Jahre zu einem primär konservativ-intensivmedizinischen Vorgehen entwickelt.

  • Das Auftreten pankreatitisspezifischer lokaler Komplikationen wie akuter und abgekapselter Nekroseansammlungen, infizierter Nekrosen oder postakuter Pseudozysten stellt die Domäne interventioneller Therapieansätze dar.

  • Im Sinne einer schrittweisen Therapieeskalation, dem sog. Step-up Approach, kommen beim Versagen der interventionellen Maßnahmen chirurgische Verfahren, vorzugsweise als minimalinvasive Eingriffe, zum Einsatz.

  • Offene chirurgische Operationstechniken, wie die offene Nekrosektomie mit additiven Konzepten wie der geschlossenen postoperativen Lavage, der Etappenlavage oder dem offenen Abdomen, bleiben heutzutage den schwersten Verläufen vorbehalten.

  • Das Outcome ist u. a. abhängig vom Ausmaß der intrapankreatischen Nekrosen.

  • Zu den Komplikationen zählen in erster Linie Störungen der endo- (Diabetes mellitus) und exokrinen Funktion (Störungen der Fettverdauung); darüber hinaus können Pankreatitisrezidive oder eine chronische Pankreatitis den Langzeitverlauf prägen.
