Operative Korrektur der Aortenisthmusstenose durch Isthmusplastik

Zusammenfassung

Die Behandlung der Aortenisthmusstenose durch Isthmusplastik hat sich in unserer Klinik bewährt. Das in zwei Modifikationen anwendbare Verfahren gestattet, jede Form der Einengung des Aortenrohres auf das gewünschte Maß zu erweitern. Die Methode ist stets anwendbar. Unter den Todesfällen sind zwei Operierte registriert, bei denen die operative Beseitigung der prästenotischen Hypertonie zu Durchblutungsstörungen im zerebralen und koronaren Gefäßapparat geführt hatte. Bei den Nachuntersuchungen konnten keine klinisch faßbaren Merkmale einer Restenosierung ermittelt werden. Alle lebenden Operierten jenseits des 14. Lebensjahres sind voll arbeitsfähig.

Summary

Isthmus-plasty has proved to be a useful method of repairing coarctation of the aorta. The method is available in two modifications and allows the widening of every type of coarctation after the coarcted segment has been resected. Among 42 patients in whom this method was used, the direct method was used in 14 without mortality, while the indirect plasty was employed in 28, with four postoperative deaths. In two of the fatal cases removal of the prestenotic hypertension had led to cerebral and coronary insufficiency. In the surviving patients no clinically demonstrable evidence of restenosis has been found and all those operated on beyond 14 years of age have had their work capacity completely restored.
